Registering
To register at the surgery it is desirable if you could bring in your current NHS Number, ask the receptionist for a registration form or download it from the link above.
At the time of registering you should also bring proof of address and a photo ID. You will be given a health questionnaire to complete, a copy of this can be downloaded from the link.
Once you have completed the registration forms an appointment will be booked for you to see the Practice Nurse/Health Care Assistant for a routine health check. If you are on any medication bring with you a repeat medications list.
At the time of registering you will be nominated a GP, who will be Dr A Dasu. This will not stop you from seeing the other clinicians and you will always be offered an appointment where possible with your named GP or other doctors if requested.
The Practice has suitable access for disabled patients.
